This agreement releases WTX Axe Throwing from all liability relating to injuries that may occur (during activity, on location, etc.). By signing this agreement, I agree to hold WTX Axe Throwing entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence. I also acknowledge the risks involved while throwing axes. These include but are not limited to muscle soreness, cuts, bruises, and splinters. I swear that I am participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity. I hereby grant WTX Axe Throwing permission to use my likeness in a photograph, video, or other digital media ("photo") in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all photos will become the property of WTX Axe Throwing and will not be returned. I hereby irrevocably authorize WTX Axe Throwing to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. By signing below I forfeit all right to bring a suit against WTX Axe Throwing for any reason. In return, I will receive entry into the axe throwing lane. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.
